HSG253 Process Isolation Lifecycle (8 Key Stages)
Compliance with HSG253 requires strict adherence to an eight-stage sequence to prevent loss of containment and human factors failures during intrusive maintenance.
1. Hazard Identification: Define toxic/flammable fluids, high pressure, thermal, and stored mechanical energy.
7. Intrusive Activity Execution: Maintain isolation boundary integrity throughout work scope.
8. Reinstatement of Plant: Controlled de-isolation, leak testing, joint control, and safety system re-arm.
HSG253 Para 21 & Figure 3 Guidance:
"Failures during the isolation and reinstatement of process plant are one of the main causes of loss-of-containment incidents, leading to major accidents. Plant reinstatement is a critical aspect; incorrect or incomplete reinstatement is likely to result in loss of containment."
Human Factors & Failure Modes (HSG253 Para 43)
Errors: Omission of bleed line isolation, failure to prove DBB cavity, reliance on unverified P&IDs.
Violations: Shortcuts due to shift pressure, single-valve isolation on live toxic streams (SBB violation).
Highest (Complete physical separation, zero risk of passing valve leaking into work area)
Mandatory for Confined Space Entry, toxic fluids (CHIP Cat 1), or long-term/extended isolations.
Category II Proved Isolation
Double Block & Bleed (DBB), Single Valve with Double Seals & Cavity Bleed, Single Block & Bleed (SBB)*
Medium-High (Valved isolation where seal tightness can be verified via vent/bleed prior to intrusion)
Baseline for flammable fluids, hydrocarbons, or steam where positive isolation is not immediately practical. (*SBB generally prohibited for hazardous fluids).
Category III Non-Proved Isolation
Double Valve, Single Valve (No vent/bleed monitoring point)
Low (No mechanism to verify valve seal integrity before breaking containment)
Non-hazardous fluids only (e.g., low-pressure utility water, non-classified services). Never on live hydrocarbon streams.
HSG253 Para 120 Rule on Single Block & Bleed (SBB):
"As a general rule, you should NOT use Single Block & Bleed (SBB) as a final isolation method for work on live plant containing hazardous substances... SBB should be used only where task-specific risk assessment has shown it to be tolerable."
